nice question. most of the times strangulated hernias are irreducible. and strangulation is the cause which results in edema of the contents of sac. so treatment is always operative in adults.

you approach through conventional incision , widen the neck of sac. the contents will readily go back which you have to prevent. check the viability of gut by its colour, glare or/and  peristalsis. if there is a doubt do hot sponging of the gut. if there is sign of life and gut recovers close the wound in layrs but dont close the skin as it is no more a clean case.
